La sindrome feto-alcolica (Fetal Alcohol Syndrome-FAS) è la più grave disabilità permanente che si manifesta nel feto esposto, durante la vita intrauterina, all’alcool consumato dalla madre durante la gravidanza.

La FAS è la conseguenza principale del consumo di alcolici durante la gravidanza, sia in maniera continuativa, quindi 1-2 unità alcoliche al giorno, e sia attraverso un elevato consumo etilico in un’unica occasione (bing drinking), più di 50 gr. al giorno, avendo la donna una tollerabilità e una resistenza inferiore rispetto all’uomo. Sicuramente ci sono dei fattori fortemente predisponenti all’alcolismo nella mamma, quali:situazioni familiari difficili,depressione, stile di vita non salutare;tuttavia anche un consumo non abituale ma frequente comel’aperitivo irrinunciabiletutte le sere, durante il periodo preconcezionale e soprattutto durante quello gestazionale,va evitato accuratamente.Nella maggior parte dei casi, questo comportamento leggero è associato anche a una cattiva alimentazione, al fumo e ad una vita sedentaria, tutti i fattori che mettono notevolmente a rischio lo sviluppo del feto.
Circa il 30% delle donne occidentali, consuma alcool
Questa percentuale di donne consuma alcol anche in gravidanza, spesso si tratta dipersone di reddito medioe nella maggior parte dei casi esse ritengono che la programmazione di una gravidanza non sia un deterrente sufficiente per il consumo di una bevanda alcolica. Il consumo dialcooldurante la gravidanzaaumenta i livelli di etanoloin circolo e, dellasostanza tossica che da esso derivata, l’ acetaldeide,che al pari delle sostanze essenziali per lo sviluppo del feto,attraversa la barriera placentare,causando peròeffetti nocivi.Il feto ha un metabolismo molto lento rispetto alla mamma e imetaboliti tossici si accumulano,interferendo con
il normale trasporto dell’ossigeno ai tessuti e agli organi in via di formazione.In particolare fino al quarto mese, quando il cervello
e la struttura fetale è più vulnerabile, l’astensione totale da alcolici deve essere assoluta. Tutto il periodo prenatale è delicatissimo e ogni fase può rendere il bambino più o meno suscettibile non solo all’alcool ma anche alle altre sostanze nocive, quali il fumo, farmaci e agenti ambientali. L’interferenza delle sostanze tossiche, derivate dall’alcool, sullo sviluppo fetale può dare vita effetti diversi.
Le malformazioni organiche
Durante il primo trimestre di gravidanza interessano particolarmente lacirconferenza del cranio (riduzione della calotta), lefessure ocularie losviluppo encefalico(microcefalia). Ad essere colpito in molti casi è lo sviluppo delnervo otticocon la compromissione della capacità visiva alla nascita. Uno degli organi a essere maggiormente compromessi nella FAS è ilcuore, soprattutto a causa dello stato di ipossia, cioè di carenza di ossigeno, cui il feto può andare incontro per l’azione tossica dell’etanolo. I disturbi dellosviluppo cognitivocausati dall’esposizione all’alcol in combinazione al fumo o sostanze farmacologiche non controllate, principalmente durante l’ultimo trimestre di gravidanza. Possono manifestarsiiperattività e deficit dell’attenzionedurante l’età prescolare, dovuti a un’interferenza nello sviluppo dell’area della memoria e dell’apprendimento.
