Generalità
L’emoglobinuria parossistica notturna(EPN), o sindrome di Marchiafava-Micheli, è una malattia caratterizzata daanemia emolitica corpuscolare,insufficienza del midollo osseoe frequentiepisodi trombotici.Questo processo distruttivo si verifica a causa della presenza di proteina superficiale DAF difettosa sui globuli rossi, che normalmente funzionano per inibire tali reazioni immunitarie. Nella gran parte dei pazienti, tuttavia, si osserva anemia emolitica cronica senza una precisa correlazione con la notte, pancitopenia, sideropenia e ricorrenti episodi trombotici. L’emoglobinuria parossistica notturna colpisce indifferentemente individui di ambo i sessi, per lo più tra 20 e 40 anni; raramente la si riscontra nell’infanzia e tra gli anziani. Non vi è famigliarità né predisposizione razziale.
Eziopatogenesi
L’emoglobinuria parossistica notturnaè determinata dalla selezione di un clone di cellule staminali ematopoietiche mutate geneticamente. La mutazione inattivante è a carico delgene PIG-A,coinvolto nel trasporto in membrana di molecoleGPI anchored,comeDAF (CD55) e MIRL (CD59),inibitori dell’ attivazione del complemento. E’ probabile, data la frequente associazione dell’ EPN con l’aplasia midollare autoimmune, che sia la pressione immunologica verosimilmente diretta controi due antigeni mancanti,a dare un vantaggio selettivo alclone patologico.La maggiore predisposizione al danno immediato del complemento comporta la comparsa di pancitopenia perlopiù cronica.
Diagnosi
La diagnosi di EPN deve essere considerata in ogni paziente che presenta i segni di emolisi intravascolare, in presenza di:
- Emoglobinuria ed emosiderinuria
- Pancitopeniain associazione con emolisi
- Grave iposideremiasoprattutto se in concomitanza di crisi emolitiche ricorrenti
- Trombosi venosespecialmente addominali ricorrenti
- Coliche addominali
- Cefaleeaccompagnate da un quadro di emolisi cronica.
Sintomi
L’emoglobinuria porta alla produzione, durante la notte e al primo mattino, di urine scure.
I pazienti possono presentare:
- Insufficienza renale
- Può essere presente ittero
In rapporto alle loro localizzazioni, le trombosi (che colpiscono il 40% dei pazienti) possono causare:
- Doloreaddominale
- Ischemiaintestinale
- Epatomegalia
- Ascite o cefalea
- Emorragiagengivale o epistassi.
Decorso e prognosi
Si tratta di una malattia cronica con sopravvivenza mediana intorno ai 10 anni. La gravità del quadro e la prognosi dipendono soprattutto da tre fattori:
- Ampiezza della popolazione patologica(EPN II/III)
- Grado di ipoplasia midollare
- Frequenza delle complicanze trombotiche e infettive.
In una piccola percentuale di pazienti la malattia regredisce completamente, mentre in un 50% si registra un discreto miglioramento della popolazione complemento-sensibile, anche se persiste la positività ai test sierologici. Nella maggioranza dei casi, però, l’evento terminale è un’ aplasia midollare globale; occasionalmente la EPN esita in leucemia mieloide in mielofibrosi.
Terapia
Laterapia trasfusionaleha effetto transitorio, in quanto il processo emolitico è rivolto anche alle emazie trasfuse e deve essere valutata, caso per caso, in base all’entità dell’ anemia. La prognosi è in relazione alla possibilità di trattamento della patologia di base. Gliandrogenisi sono rivelati solo saltuariamente utili mentre i corticosteroidi a dosi elevate possono arrestare una crisi emoglobinurica. Letrasfusioni GR lavaticorreggono momentaneamente l’anemia.Attualmente la terapia conanticorpi monoclonali anti- CD55( Rituximab) ha dato risultati significativi nel controllo delle crisi parossistiche.
