Cos’e l’endometriosi?
L’endometriosi colpisce nel mondo il 10% delle donne in età riproduttiva, è determinata dall’accumulo anomalo di cellule endometriali al di fuori dell’utero. Eppure di questa patologia se ne parla ancora troppo poco.
Cosa occorre sapere
Si stima che, nel mondo, 176 milioni di donne in età fertile soffrano di endometriosi. In Italia la cifra si attesta sui 3 milioni, almeno per quanto riguarda i casi conclamati. Infatti, c’è ancora chi è resistente a parlarne per via del tabu socio-culturale. Dopo 15 anni di assenza, sono stati aggiornati i cosiddetti LEA, livelli essenziali di assistenza, delle terapie che il Servizio Sanitario Nazionale garantisce di fornire gratuitamente in tutte le regioni. E’ stato previsto anche l’inserimento dell’endometriosi nell’elenco delle patologie croniche invalidanti, riconoscendo, di conseguenza, alle pazienti il diritto di usufruire in esenzione di alcune prestazioni specialistiche di controllo.
Aspetti clinici
L’endometriosi è una malattia devastante, fortemente limitante per chi ne soffre. Si tratta di una patologia benigna dell’epoca fertile della donna, che deriva dal reperimento di mucosa endometriale, che normalmente riveste la cavità uterina, la quale si sfalda durante le mestruazioni, al di fuori della sua sede naturale, ovvero l’utero. Tale tessuto e tali cellule durante il ciclo, refluiscono attraverso le tube, cadono all’interno della cavità addominale e, in alcune donne, trovano il modo di attecchire su varie strutture, come ovaie e peritoneo, che riveste tutti gli organi all’interno dell’addome. O, ancora, migrando in organi vicini all’apparato genitale, essi si diffondono su intestino, ureteri, vescica. Una volta che il tessuto endometriale si è impiantato in sede ectopica, al di fuori dell’utero, risponde agli stimoli ormonali, per cui gli estrogeni che sono gli ormoni che la donna produce in età fertile, ne permettono la sopravvivenza e ne stimolano la proliferazione.
Si possono avere figli?
La sterilità è presente in circa il 30-35% delle pazienti desideroso di avere figli affette da endometriosi. Ma non è corretta etichettare una paziente che ne soffre come “una donna che non potrà avere gravidanze”, perché non è così. La malattia può incidere sia per quanto riguarda la presenza di un ambiente biologico poco favorevole alla fecondazione, sia relativamente alle alterazioni morfologiche e funzionali a carico di tube, ovaie, a causa di inappropriati interventi chirurgici. Bisogna però essere fiduciosi perché la ricerca sta compiendo passi promettenti anche su questo fronte. L’utilizzo di tecniche meno aggressive sempre laparoscopiche, come l’utilizzo di laser CO2, permette di offrire un trattamento completo, ma meno traumatico per le ovaie e quindi potenzialmente meno dannoso.
Diagnosi
Si dice spesso che è difficile effettuare la diagnosi di endometriosi. La diagnosi non risulta affatto complessa qualora la persona racconti tutta la storia in modo dettagliato e il medico sappia ascoltarla con attenzione. Esistono inoltre manovre che si possono eseguire durante la visita ginecologica e che bene evidenziano se si configura un sospetto di endometriosi. Attualmente la metodica strumentale d’elezione per diagnosticare l’endometriosi è l’ecografia pelvica transvaginale. A seconda del quadro clinico e dello stadio di diffusione della malattia, si possono utilizzare metodiche di secondo livello, quali la risonanza magnetica o indagini del tratto gastrointestinale, come la colonoscopia o la cistoscopia, nel caso di interessamento vescicale.
In certi casi possono trascorrere dai 7 ai 9 anni da quando si inizia a soffrire di endometriosi a quando viene diagnosticata, ma ciò dipende da tanti fattori, non dalle difficoltà delle analisi. Da un lato, il ritardo diagnostico può derivare da una scarsa conoscenza da parte dei medici della patologia e del modo di metterla in evidenza, dall’altra parte, può accadere che la paziente tenda a celare determinati sintomi, come i dolori durante il ciclo mestruale o un rapporto sessuale: il fatto di parlarne per alcune persone, rappresenta ancora un tabù.
Cause
Per quanto riguarda le possibili cause della malattia il quadro è ancora tutto da definire. In questi anni la ricerca si sta concentrando sui possibili fattori genetici. Si è riscontrata, infatti, una familiarità: non è infrequente trovare mamma, figlia e sorella affette da endometriosi. Probabilmente, si vanno a delineare alcune frequenze geniche che potrebbero essere alla base della predisposizione a sviluppare l’endometriosi
Terapie moderne
Le terapie moderne possono essere sia di tipo farmacologico, sia chirurgico. Nel primo caso, si tratta di terapie, che sostanzialmente, si basano sull’utilizzo di ormoni, quindi pillole estroprogestiniche o solo progestiniche o ancora della funzione degli estrogeni . Sono efficaci, possono proseguire per parecchio tempo e presentano bassi costi, tuttavia implicano che, durante l’assunzione non si possa cercare contemporaneamente la gravidanza. La ricerca è orientata a trovare un farmaco che contrasti l’endometriosi e, al tempo stesso, permetta alla donna di rimanere incinta. Esistono anche delle terapie più aggressive, che però non possono essere protratte per lungo tempo, dal momento che mimano una sorta di “menopausa farmacologica” con tutti gli effetti collaterali indesiderati. Relativamente agli interventi chirurgici, invece oltre il 90% di essi viene condotto in modo meno invasivo, per via laparoscopica. Come nel caso della terapia farmacologica, è difficile riuscire ad adottare un protocollo rigido, valido per tutte le pazienti, ma occorre modulare e personalizzare i trattamenti.
