Definizione
Con il termine “glomerulosclerosi focale e segmentaria” si definsce una sindrome clinico-patologica, definita anche podocitopatia, caratterizzata da proteinuria, frequentemente superiore ai 3 g nelle 24 ore, ipertensione arteriosa, progressiva insufficienza renale, e frequente resistenza ai corticosteroidi. Il termine focale deriva dal limitato numero di glomeruli interessati dalle lesioni sclerotiche nella corticale; inoltre, in ognuno di essi la lesione è segmentaria perché è coinvolta solo una parte della matassa glomerulare.
Epidemiologia
La glomerulosclerosi focale e segmentaria colpisce sia i bambini sia gli adulti. La fascia di età maggiormente colpita va dai 10 ai 25-30 anni.
Eziopatogenesi
La glomerulosclerosi focale e segmentaria è idiopatica nella maggior parte dei casi; ha un rapporto di associazione (ma non causa-effetto) con:
- Rene trapiantato
- Monorene
- Uropatie ostruttive e da reflusso
- Obesità di III classe
- Infezione da HIV
- Abuso di eroina
- Rarissime cause ereditarie
Sintomatologia
L’età di inizio della malattia è prevalentemente al di sopra dei 6 anni nei bambini e nel secondo decennio di vita negli adulti. Il quadro clinico di esordio della malattia è caratterizzato da una sindrome nefrosica che rappresenta la manifestazione più frequente. Nel 10-30% dei casi si può osservare nella fase iniziale, solo proteinuria lieve associata a ipertensione, che è molto frequente quando è presente un’insufficienza renale cronica.
Dati di laboratorio
La proteinuria giornaliera può variare dai 1-2 g sino ai 15-20 g nelle 24 ore. E’ quasi sempre non selettiva. Può essere presente microematuria. La funzione renale può essere normale o alterata con valori di creatinina che non superano 2-3 mg/dl.
L’ecografia renale può evidenziare un volume normale o lievemente ridotto dei reni, con iperecogenicità della corticale.
Diagnosi
La comparsa di una sindrome nefrosica corticoresistente associata a ipertensione arteriosa deve orientare verso il sospetto di una glomerulosclerosi focale e segmentaria. La biopsia renale è l’unica indagine per effettuare una corretta diagnosi.
Complicanze
Si possono considerare due tipi di complicanze: quelle legate alla malattia e quelle conseguenti all’uso dei farmaci.
Le complicanze legate alla malattia sono:
- Uremia cronica terminale che si osserva nel 60% dei pazienti entro 10-15 anni dall’inizio della malattia.
- Trombosi della vena renale o di altri distretti per la sindrome nefrosica che non risponde alla terapia.
- Infarto del miocardio come conseguenza di una aterosclerosi accelerata.
- Infezioni batteriche.
Complicanze legate all’uso dei farmaci immunosoppressori sono:
- Ipercorticismo da uso prolungato di prednisone.
- Effetto tossico dei farmaci immunosoppressori come le ciclofosfamide, che può determinare disfunzioni gonadiche quali azoospermia nei maschi e l’amenorrea nelle donne.
- Effetto nefrotossico della ciclosporina quando la somministrazione supera il 12-38 mesi.
Decorso clinico e prognosi
Il decorso clinico della malattia è caratterizzato da una progressiva riduzione della funzione renale e circa il 60% dei pazienti sviluppa un’ uremia terminale entro 10-15 anni dall’inizio della malattia. Alcuni pazienti sviluppano una forma maligna della malattia e presentano l’uremia terminale entro 2-3 anni.
Fattori prognostici sfavorevoli sono principalmente la sindrome nefrosica, l’insufficienza renale, la scleroialinosi dei glomeruli e gli infiltrati tubulo interstiziali. Un fattore prognostico attendibile è rappresentato dalla risposta alla terapia corticosteroidea.
Terapia
L’evoluzione verso l’insufficienza renale cronica è molto frequente in questi pazienti e il migliore segno prognostico è rappresentato dalla risposta alla terapia. La terapia iniziale specifica può consistere nella somministrazione del corticosteroide (prednisone).
