Listeriosi: infezione da listeria

Che cos’è la listeriosi?
La listeriosi è definita dall’OMS una malattia relativamente rara, ma preoccupante per l’elevato tasso di letalità (20- 30% ) tra i pazienti ospedalizzati. La listeriosi è una malattia batterica causata da listeria monocytogenes, le cui manifestazioni cliniche sono polimorfi. La listeria monocytogenes è un bacillo aerobio Gram + non sporigeno e mobile. La forma clinica più usuale della malattia è la batteriemia, che si manifesta spesso durante la gravidanza, nel corso della quale, pur non provocando listeriosi a esito drammatico nella madre, si possono verificare aborto, parto prematuro, danni al feto e gravi forme settiche perinatali .
Eziologia
La listeria monocytogenes è un agente patogeno che si trasmette con il cibo e può causare infezioni gravi, soprattutto nelle donne gravide e nei soggetti immunodepressi. La listeriosi può far seguito all’ingestione di cibo contaminato: listeria può essere presente in cibi lavorati e non lavorati, come formaggi molli, preparazioni gastronomiche a base di carne, hot dog, latte e insalate fredde, pesce, verdure crude, latte non pastorizzato e latticini, burro.
Quali sono i sintomi della listeriosi?
La listeriosi provoca numerose sindrome cliniche gravi, le più comuni sono meningite e setticemia.
- Gastroenterite: può svilupparsi entro 48 ore dall’ingestione di cibo contaminato contenente un’elevata carica batterica. La diagnosi di listeriosi va presa in considerazione negli episodi epidemici di gastroenterite, quando l’esame colturale per altri patogeni probabilmente in causa risultino negative.
- Batteriemia: i pazienti presentano febbre, brividi, mialgie e artralgie. I reperti neurologici dei segni meningei possono suggerire la diagnosi. L’endocardite è rara ed è accompagnata da tassi di mortalità del 35-50%.
- Meningite: provoca circa il 15-10% dei casi di meningite acquisiti in comunità negli adulti. La triade clinica di febbre, rigidità nucale anomalie del comportamento è presente nel 43% dei casi, i deficit neurologici focali nel 16-37 % e le convulsioni nel 4-17% dei pazienti. Nell’ nadulto defedato è stata recentemente descritta un’ encefalite da listeria caratterizzata dalla presenza di plurime lesioni focali variamente distribuite, interessanti soprattutto la sostanza bianca sottocorticale.
- Infezione nelle donne gravide e nei neonati: in gravidanza la listeriosi è un’infezione grave. Le pazienti presentate di solito batteriemia e malattia febbrile non specifica con mialgie e artralgie, dolore dorsale e cefalea. L’interessamento del SNC è raro. L’infezione si sviluppa nel 70-90% dei feti di madri infette e quasi la metà dei feti infetti muore. Il rischio può ridursi con il trattamento pre-parto. Di solito, dopo il parto le donne infette migliorano. Una violenta infezione fetale (Granulomatosis infantiseptica) è caratterizzata da microascessi miliari e granulomi, più spesso a livello della cute, del fegato e della milza.
Diagnosi
Per una diagnosi tempestiva è necessario che la malattia venga presa in considerazione nei gruppi a rischio: donne gravide, anziani, neonati, soggetti immunodepressi (per esempio paziente sottoposto a trapianto, pazienti affetti da neoplasie maligne, ecc.), soggetti con patologie mediche croniche di base per esempio (alcolismo, diabete). La diagnosi di listeriosi è posta quando il microrganismo viene isolato da una sede sterile, come sangue, liquor o liquido amniotico. La listeriosi va sospettata in caso di minacce d’aborto in madri apparentemente sane o affette da febbre di natura indeterminata; nell’ infezione gravidica o neonatale l’indagine colturale sul più ampio numero di campioni biologici (liquido amniotico, placenta, tampone cervicale), deve essere comunque immediatamente seguito da un’ antibioticoterapia aggressiva.
Come si cura la listeriosi?
L’antibiotico di prima scelta per la terapia della listeriosi e l’ampicillina, da associare all’ aminoglicoside in caso di meningite. La terapia andrebbe comunque impostata sulla scorta dei risultati dell’antibiogramma, se è possibile; nei pazienti allergici alla penicillina la migliore alternativa terapeutica sembra essere il cotrimossazolo. Il germe risulta spesso sensibile anche all’eritromicina e alle tetracicline, mentre è tipica la resistenza alle cefalosporine, che non vanno pertanto utilizzate nella terapia della listeriosi. La malattia è soggetta a notifica obbligatoria.
