Narcisismo e istrionismo: disturbi della personalità

Il narcisismo è un disturbo caratterizzato dalla grandiosità, una grandiosità che non ha giustificazione. Il soggetto si sente speciale ed eletto, ha la necessità di essere ammirato. Il nucleo del narcisista è la mancanza di empatia. La persona ha un senso grandioso di importanza, può avere delle fantasie di successo, di potere, di fascino ecc.. Crede di essere:
- speciale e quindi tende ad aggregarsi a persone che ritiene di rango elevato;
- richiede eccessiva ammirazione;
- pensa che tutto gli sia dovuto;
- si aspetta che tutti i suoi bisogni vengano soddisfatti;
- si aspetta di venire trattato in maniera differente dagli altri.
La parte oscura del narcisismo è la parte relazionale: cioè l’empatia, il narcisista non è interessato all’altra persona in maniera autentica ma solo perché gli può portare utile in ambito narcisistico (es. una persona che lo ammiri, che lo sostenga, che lo faccia brillare di luce diversa). Imposta delle relazioni basate sullo sfruttamento, che non necessariamente ha a che fare con il denaro.
Prende dagli altri quello che gli serve, non si riconosce con i sentimenti degli altri, non nutre il minimo interesse ed è invidioso, la vergogna e la rabbia sono degli aspetti importanti della personalità narcisistica.
E’ arrogante, presuntuoso ed esibizionista.
IL NARCISISTA MASCHERATO
Non tutti i narcisisti presentano le caratteristiche sopra descritte, c’è un tipo di narcisista un po’
più mascherato che non immediatamente rivela questi aspetti grandiosi ma solo a lungo andare oppure in un rapporto più impegnativo. La definizione del DSM non contempla questa tipologia di narcisisti per cui, basandosi solo sulla definizione, i soggetti appartenenti a quest’ultima categoria possono non essere scoperti. Gli anglosassosi li definiscono covers. Il soggetto cover è mascherato, può avere degli atteggiamenti umili, sottomessi, può essere carino e nascondere questo senso di importanza ed il bisogno di essere ammirato.
ISTRIONISMO
L’istrionico per molti aspetti somiglia al narcisista. Questi due disturbi appartengono allo stesso cluster quindi è normale che abbiano dei caratteri comuni. Bisogna fare una diagnosi differenziale: gli istrionici hanno come il narcisista il bisogno di attenzioni, si presentano in modo seduttivo ma hanno un’espressività esagerata. L’istrionico è a disagio quando non è al centro dell’attenzione; è inutilmente seduttivo, il suo rapporto con gli altri passa sempre attraverso la seduzione anche quando la seduzione non è richiesta, quindi mostra un comportamento sessualmente provocante anche in contesti in cui non è utile oppure nei confronti di persone verso cui l’istrionico è indifferente. Gli istrionici non hanno alcuna consapevolezza di questa
seduttività manifesta; alcune volte si ritrovano in difficoltà quando gli altri rispondono in maniera sessualmente coerente e ne rimangono offesi, perché c’è questa assenza di consapevolezza. L’istrionico non si rende conto di questo suo atteggiamento seduttivo perché non sta cercando sesso ma:
- attenzione
- affetto
- calore
- intimità.
L’istrionismo si associa a una certa immaturità della personalità.
CARATTERISTICHE
Espressione delle emozioni molto superficiale e mutevole;utilizzazione dell’aspetto fisico per attirare l’attenzione, lo stile dell’eloquio è impressionistico (privo di dettagli), cioè gli istrionici non sono in grado di descrivere una persona utilizzando dei termini razionali, ma usando le loro impressioni. Ad es..l’istrionico davanti al test di Rorschach: posto davanti ad una tavola potrà definirla orribile, paurosa, limitandosi quindi a descrivere le sue impressioni. Magari nella tavola vedrà un mostro, ma sarà incapace di individuare la testa o il corpo.
USO DI IPERBOLI
Stile cognitivo perché l’istrionico rapporta tutto a se stesso e non ha una visione chiara e obiettiva della realtà, ha solo una visione molto soggettiva basata sulle impressioni emotive. Di conseguenza drammatizza, è teatrale, ha un’espressione esagerata delle emozioni, è suggestionabile (le prime pazienti di Freud erano tutte isteriche, l’isterico è il disturbo istrionico). Il soggetto è suggestionabile perché crede a tutto e si lascia influenzare dalle opinioni degli altri e dalle mode. E’ attirato tutto ciò che è brillante ed eccitante. L’istrionico ha un significato negativo, rimanda ad una dimensione teatrale: l’istrione è un attore che carica troppo la parte, che carica il personaggio di gesti inutili, esagera. Il soggetto istrionico se sta male sta morendo; se sta bene sta in paradiso. Non ha un equilibrio emotivo né razionalità e drammatizza le emozioni. Spesso il disturbo istrionico si associa con tendenze ipocondriache oppure con tendenza alla somatizzazione.
