Definizione
IlMycoplasma pneumoniaeè responsabile della maggior parte delle polmoniti da patogeni atipici e l’andamento della malattia da esso causata può essere considerato il prototipo dellepolmoniti atipiche. IlMycoplasma pneumoniae (agente di Eaton) si trasmette percontagio diretto,spesso in una comunità (famiglia, scuola, caserme).
Epidemiologia
La polmonite daMycoplasma pneumoniaesi sviluppa prevalentemente in microepidemie, spesso a carattere familiare. Non sono comunque infrequenti i casi sporadici. Il microorganismo è veicolato nella mucosa delle prime vie aeree e l’infezione si propaga per contagio interumano, tramite le goccioline di saliva in sospensione. Sono coinvolti spesso i bambini in età scolare, i quali poi possono trasmetterlo agli altri membri della famiglia attraverso le goccioline volatili. La polmonite causata daMycoplasma pneumoniaeè spesso presente nei centri di addestramento per reclute militari, anche se solo una piccola percentuale di soggetti portatori sviluppa una malattia clinicamente rilevabile.
Fisiopatologia
L’infezione da micoplasma pneumoniae è relativamente frequente e ha unacaratteristica di benignità. La lesione principali consiste in un infiltrato flogistico, costituito in prevalenza da polimorfonucleati, e si estende dal connettivo peribronchiale lungo i vasi ematici e linfatici nei setti interlobulari e nel piccolo interstizio. Le lesioni non sono mai tali da interferire in maniera significativa con lafunzionalità respiratoria, e perciò a differenza delle polmoniti batteriche non si presenta compromessa in questa malattia.
Manifestazioni cliniche
Il periodo diincubazione della malattia è di 2-3 settimane.Il paziente colpito da polmonite è spesso un giovane soggetto adulto, generalmente di sesso maschile e in precedenza del tutto sano.
Il paziente presenta :
- Mal di gola
- Tosse stizzosacon espettorato scarso
- Dolore auricolare, causato dalla propagazione della flogosi nell’orecchio medio fino alla membrana timpanica
- Febbre(38,5-39°)
- Successivamente la malattia si propaga l’interstizio polmonare,non determinando tuttavia una sintomatologia rilevante.
Sono presenti sintomi costituzionali quali:
- Malessere generale
- Cefalea frontale
- Atromialgie diffuse.
Diagnosi
La diagnosi di polmonite da Mycoplasma si basa essenzialmente su dati clinici ed epidemiologici. La malattia colpisce spesso più membri di una stessa famiglia e determina una sintomatologia per lo più scarsa. Un utile contributo diagnostico può derivare dalla ricerca delcrioagglutinine, presenti in più della metà dei pazienti colpiti a partire dalla 1ª-2ª settimana dall’esordio clinico e tendenti a ridursi entro la 4ª-6ª settimana. La diagnosi può essere agevolata dalla ricerca dianticorpi anti- mycoplasmafissanti il complemento.
L’esame radiologico del toraceè di decisiva importanza per la diagnosi di certezza di questa polmonite.
Decorso, prognosi e terapia
Questa forma di polmonite ha unandamento benignoe nella maggior parte dei casi si risolve spontaneamente nel volgere di 3-4 settimane qualora venga instaurata una terapia antibiotica efficace costituita in prima istanza dalletetraciclinee daimacrolidi.
