Endocardite infettiva

Definizione
L’endocardite infettiva (EI) è l’infezione della superficie endocardica del cuore, che colpisce prevalentemente le valvole, ma che può interessare anche difetti dei setti, corde tendinee, endocardio murale, shunt tra arterie e vene o tra arterie e arterie, coartazione aortica.
Vegetazione:è la lesione più caratteristica dell’endocardite infettiva. Massa di piastrine, fibrina, microclonie di microrganismi e scarse cellule infiammatorie.
Epidemiologia
Si stima che l’endocardite infettiva abbia un’incidenza annuale nei paesi occidentali di circa 4-7 casi 100.000 abitanti e che colpisce prevalentemente i soggetti anziani. Le principali condizioni predisponenti allo sviluppo di endocardite include ilconsumo di sostanze stupefacentiper via endovenosa, levalvulopatie degenerative, la presenza didispositivi intracardiacio diprotesi valvolarie lecardiopatie congenite. La cardiopatia reumatica è ormai di rado riscontro nelle nostre latitudini, è invece un fattore di rischio molto frequente nei paesi in via di sviluppo.
Il 16-30% di tutti i casi di endocarditi infettive interessa le valvole protesiche, per le quali il rischio di infezione è massimo durante i primi 6-12 mesi dopo l’intervento chirurgico. Il rischio infettivo, peraltro non è sostanzialmente differenze tra protesi meccaniche e protesi biologiche. Infine, il 25-35% dei casi di endocardite su valvola nativa nei paesi industrializzati è associata alle cure sanitarie (endocardite nosocomiale).
Classificazione
Un tempo l’endocardite era classificata, in base al suo decorso, comeacutaesubacutae quest’ultima forma era di gran lunga la più comune. Oggi la forma ad andamento acuto è divenuta sempre più frequente, soprattutto a seguito dell’abuso di droghe per via venosa. L’endocardite acuta insorge frequentemente in pazienti senza alterazioni cardiache preesistenti e si comporta come una malattia a rapida evoluzione causata da microrganismi altamente patogeni, in primo luogo loStaphylococcus aureus.E’ caratterizzata dafebbre elevata, segni di tossicità sistemica, scompenso cardiaco rapidamente progressivo e, se non trattata, morte nel giro di giorni o settimane.
Eziologia
L’endocardite infettiva varia a seconda della localizzazione della vegetazione (valvola nativa, valvola protesica, pacemaker)e del contesto epidemiologico (endocardite comunitaria, nosocomiale o nel paziente tossicodipendente per via iniettiva). Nel paziente convalvola nativala prima causa di endocardite infettiva comunitaria e nosocomiale èStaphylococcus aureus,che in generale costituisce il più comune agente eziologico dell’endocardite infettiva. Nelle infezioni acquisite in comunità gli streptococchi.Gli streptococchi del cavo orale, compresa gran parte di quelli del gruppo viridans,rappresentano un’altra causa importante, secondo alcune casistiche addirittura (40% dei casi di EI comunitaria su valvola nativa). Merita particolare attenzione loStreptococcus bovis (attualmente denominato Streptococcus gallolyticus) appartenente al gruppo D degli streptococchi, in quanto una batteriemia rende necessaria la realizzazione di una colonscopia per escludere la presenza di una lesione a livello della mucosa del colon (neoplasia o polipi). Nelle endocarditi nosocomiali su valvola nativa S. aureus ed enterococchi rimangono principali agenti biologici, mentre, a differenza delle forme comunitarie, viene a mancare il ruolo prominente degli streptococchi del cavo orale. Lo spettro eziologico dell’endocardite su valvola protesica ( PVE) sono classificate ad insorgenza precoce, intermedia o tardiva a seconda della latenza di inizio dei sintomi dell’endocardite dopo l’intervento chirurgico
- Esordio precoce: quelle infezioni che si verificano entro 60 giorni dalla chirurgia e di solito si tratta di infezioni acquisite in ambito sanitario. Lo Staphylococcus aureus è il patogeno più comunemente responsabile.
- Esordio intermedio: si manifesta da 60 a 365 giorni dopo la sostituzione della valvola cardiaca. Il profilo microbiologico delle infezioni durante questo periodo è variabile e comprende sia infezioni nosocomiali
che acquisite in comunità; in questo gruppo il patogeno più comunemente isolato è rappresentato dagli
stafilococchi coagulasi-negativi.
Nei pazienti portatori dipacemaker o defibrillatore cardiaco impiantabile, l’endocardite costituisce una complicanza quasi sempre di origine stafilococcica (S. aureus e CoNS). Nei pazienti tossicodipendenti per via iniettiva l’agente eziologico principale è lo S. aureus, responsabile del 75-80% delle infezioni del cuore destro. Da ultimo, permane una quota significativa di casi, che può variare dal 5 al 10%, nei quali non è possibile identificare l’agente eziologico (endocardite emocolture negative). In questi casi è prioritario escludere cosiddetti falsi negativi, in cui l’impiego recente o concomitante di terapia antibiotica riduce significativamente la sensibilità delle emocolture.
Manifestazioni cliniche
Le manifestazioni cliniche più frequenti dell’endocardite infettiva sono: lafebbre,presente nel 80 90% dei casi e spesso associata a sintomi costituzionali,malessere generale,astenia, atromialgie,calo ponderale.In una quota rilevante di pazienti è possibile apprezzare unsoffio cardiaco di recente comparsa, o nel caso di pregressa valvulopatia, un significativo cambiamento delle caratteristiche del soffio presente in precedenza (20-50% dei casi). Nelle forme più gravi, inoltre, possono comparire i segni delloscompenso cardiaco congestizioo, per effetto dell’interessamento del tessuto di conduzione,turbe del ritmo.In circa il 20-50% dei casi si possono osservare manifestazioni cliniche dovuta a fenomeni diembolizzazione settica.La presenza di vegetazione di grandi dimensioni (> a 10 mm), mobili e/o localizzate in sede mitralica e l’infezione da S. aureus ne costituiscono i principali fattori di rischio.
Le complicanze a carico del sistema nervoso centrale, in particolare, condizionano spesso la prognosi del paziente e includono ictus ischemico, l’emorragia intraparenchimale e la rottura di aneurismi micotici.
Sintomi muscolo-scheletrici: mialgie e atralgie, dolore dorsale.
Interessamento renale: glomerulonefrite causata da deposizione di immunocomplessi, tra i quali inoduli di Osler, (noduli dolenti in corrispondenza dei polpastrelli delle dita) e lemacchie di Roth(emorragie retiniche concentro pallido) visibili all’esame del fundus. Per quanto manifestazione aspecifiche, è inoltre possibile apprezzare all’esame obiettivo la presenza displenomegalia(15-50%) dei casi e o di ippocratismo digitale (15 20%).
Esami di laboratorio
Gli esami di laboratorio spesso documentano anemia iporigenerativa da malattia cronica, talora associata a leucocitosi neutrofila. Molto frequente è il rialzo degli indici di floglosi (VES,PCR), i cui livelli sierici tuttavia non correlano con l’efficacia delle terapia antibiotica. Infine, per effetto dell’attivazione del sistema immune, è possibile riscontrare positività del fattore reumatoide e/o consumo delle frazioni solubili del complemento ( C3, C4).
Trattamento dell’endocardite infettiva
Terapia antibiotica
Gli antibiotici devono essere battericidi a dosi alte per via intravenosa. La durata della terapia antibiotica deve essere prolungata tra le 4-6 settimane e almeno per 6 settimane nel caso di valvole protesiche. In tutti i casi di endocardite infettiva, ancor prima che venga identificato l’agente eziologico e il suo spettro di sensibilità antibatterica è necessario intraprendere tempestivamente una terapia antibiotica empirica ad ampio spettro al fine di coprire microrganismi più probabilmente responsabili dell’infezione.
Trattamento chirurgico
In circostanze particolari, in aggiunta alla terapia antibiotica, deve essere attentamente valutata l’opportunità di ricorrere all’intervento chirurgico.In particolare, la comparsa di segni e sintomi discompenso cardiaco,l’evidenza di infezione non controllata e la prevenzione dei fenomeni embolici in pazienti ad alto rischio impongono una valutazione cardiochirurgica.
Prognosi dell’endocardite infettiva
La mortalità intra-ospedaliera dei pazienti con EI varia dal 15 al 30%. La rapida identificazione dei pazienti ad alto rischio di morte può garantire l’opportunità di cambiare il corso della malattia (ad esempio, intervento chirurgico in emergenza-urgenza) e migliorare significativamente la prognosi. Sono i quattro i fattori principali che influenzano la prognosi del paziente affetto da endocardite: le caratteristiche del paziente, la presenza o assenza di complicanze cardiache e non cardiache,l’agente eziologico e i dati ecocardiografici.
I fattori associati all’aumento della mortalità sono:
- Età avanzata
- Protesi valvolare
- Comorbidità gravi
- Diabete mellito
- Infezione nosocomiale
- S. aureus
- Bacilli gram-negative o funghi multiresistenti
- Scompenso cardiaco
- Complicanze neurologiche gravi
- Insufficienza renale
- Interessamento della valvola aortica
- Complicanze perianulari
- Insufficienza valvolare severa(mitrale o aortica)
