Terapia colite ulcerosa
La terapia medica della colite ulcerosa è mirata principalmente a:
- controllare e far regredire l’infiammazione delle fasi acute

La forma lieve-moderata di rettocolite ulcerosa non richiede il ricovero ospedaliero; i farmaci di prima scelta, per la loro documentata efficacia e per la bassa probabilità di effetti collaterali, sono gli aminosalicilati, di cui la mesalazina è la molecola più utilizzata. Nei pazienti refrattari alla terapia con salicilati è necessario ricorrere ai corticosteroidi tradizionali o ai più recenti corticosteroidi a bassa biodisponibilità, entrambi disponibili sia
per il trattamento per os sia per il trattamento per via rettale.
Forme gravi
La frequenza delle forme gravi non è elevata e sta ulteriormente riducendosi per effetto dell’ impostazione terapeutica più tempestiva ed efficace. Sono le forme (soprattutto quelle fulminanti) nelle quali è massimo il rischio che il paziente sviluppi un megacolon tossico e che le sue condizioni cliniche evolvano verso la sindrome da risposta infiammatoria sistemica (SIRS) e/o
la Multiple Organ Failure Syndrome (MOFS).
Terapia di mantenimento
In generale, la terapia di mantenimento è indicata in tutti i pazienti con colite ulcerosa in cui si sia riusciti a indurre la remissione dopo una fase di acuzie, indipendentemente dalle estensione di malattia. È infatti dimostrato che la somministrazione orale continuativa di aminosalicilati a dosaggio superiore a 1,6 g/die
è in grado di ridurre significativamente il rischio di riaccensione rispetto al trattamento con placebo. La terapia di mantenimento con un corticosteroidi non deve essere presa in considerazione per la scarsa efficacia e l’elevato numero di effetti collaterali.
Terapia chirurgica
Circa il 15-20% dei pazienti portatori di rettocolite ulcerosa necessità di un intervento chirurgico a causa della comparsa di complicanze o per la mancata risposta alla terapia medica. La scelta di candidare all’intervento elettivo un paziente deriva dalla mancata risposta al trattamento medico, dalla dipendenza dall’uso di steroidi, dalla necessità di sospendere la terapia a causa di effetti collaterali o in presenza di displasia o cancro. In altre circostanze l’intervento si rende necessario con carattere di urgenza per la comparsa di complicanze come il megacolon tossico, l’emorragia intestinale o la perforazione.
