Gomito
Il gomito è la regione dell’arto superiore in cui si compie l’articolazione tra braccio e avambraccio (cioè tra omero, ulna e radio); la regione posteriore del gomito, l’angolo sporgente che si forma piegando l’avambraccio.
Cause del dolore al gomito
Il gomito è soggetto a traumi e sforzi. Il dolore può essere determinato da cause tendinee e cause scheletriche, cioè dell’articolazione. Molto frequentemente la principale causa è epicondilite, un processo infiammatorio che colpisce i tendini, patologia comunemente nota come “gomito del tennista. Meno frequente, invece, è la cosiddetta epitrocleite, un’infiammazione dei muscoli dell’epitroclea, nell’area sotto il gomito, che rende dolorosi, e quando è molto grave, anche impossibili alcuni movimenti semplici come ruotare la maniglia della porta,stringere la mano, o utilizzare alcuni strumenti.

- borsite
Il gomito del tennista
Si tratta di un disturbo degenerativo di usura dei tendini. Se non curato, peggiora con il passare del tempo. Si tratta di una tipica patologia da eccessivo carico funzionale, anche detta over-use syndrome, una condizione che si verifica in coloro che compiono sforzi ripetuti del medesimo tipo tra cui sportivi (tennisti, ma anche schermidori, tiratori con la pistola ecc..)
Quali categorie di lavoratori può colpire?
Il dolore al gomito non riguarda soltanto i giocatori di tennis ma anche i lavoratori manuali che fanno movimenti ripetitivi di torsione del gomito, polso e mano e che sovraccaricano i cosiddetti tendini estensori.Per esempio può soffrirne chi utilizza cacciaviti inglesi, i cuochi, i baristi che stanno alla macchina del caffè, gli imbianchini, i muratori. Può colpire anche chi sta molte ore davanti al computer utilizzando tanto il mouse assumendo una postura scorretta.
Sintomi
I sintomi principali sono:
- dolore e limitazione articolare: il paziente non riesce a fare movimenti fondamentali per la vita di relazione, il gomito perde l’estensione e rimane un pò flesso.
Cosa fare per diminuire il dolore?
Oltre a sospendere l’attività che ha provocato il dolore, può dare sollievo l’applicazione di una borsa di ghiaccio per 15 minuti diverse volte al giorno. Inoltre è utile indossare un’apposita fascetta di tipo elastico regolabile da posizionare cinque centimetri sotto il gomito che riesce ad evitare la trazione dei tendini.
Diagnosi
L‘esame clinico è sufficiente per capire, attraverso i movimenti e la reazione dolorosa del paziente, se si tratta di “gomito del tennista”. Per completare la diagnosi può essere d’aiuto l’ecografia.
Terapia
Può essere efficace sottoporre il paziente a un ciclo di infiltrazioni: con una siringa si inietta, e dunque si infiltra, nell’area dolente un anestetico eventualmente accompagnato da un antinfiammatorio o da un cortisonico. Se, i sintomi non migliorano, è necessario l’intervento chirurgico che è risolutivo.
Prevenzione
Sospendere di tanto in tanto, se è possibile, un certo tipo di attività ripetitiva per non sovraffaticare i tendini. Poiché si tratta di una malattia degenerativa, però, molto dipende dal grado del disturbo: se è in stato avanzato, purtroppo non è sufficiente a fare scomparire i sintomi.
