Che cos’è l’emocromo?
L’emocromo, anche noto come esame emocromocitometrico che consente di valutare alcuni parametri fondamentali per la salute dell’organismo, come:
–la conta dei globuli rossi,che indica la capacità del sangue di trasportare l’ossigeno ai tessuti
–la conta dei globuli bianchi, che rappresentano il sistema immunitario del corpo e possono indicare la presenza di infezioni o malattie autoimmuni
–la conta delle piastrine, che sono fondamentali per la coagulazione del sangue
–l’emoglobina e l’ematocrito, che riflettono la quantità di globuli rossi nel sangue e la loro capacità di trasportare l’ossigeno.
L’emocromo può quindi essere utile per diagnosticare e monitorare diverse patologie, come anemia, infezioni, malattie del sangue e malattie cardiovascolari. Inoltre, può essere richiesto anche come strumento di screening per valutare lo stato di salute generale del paziente.
Valori normali emocromo
I valori normali dell’emocromo variano in base all’età, al sesso e alle condizioni fisiologiche dell’individuo, ma di seguito sono riportati i valori di riferimento approssimativi per un adulto sano:
–Globuli bianchi(leucociti): da 4.000 a 11.000 cellule/mcl
–Globuli rossi(eritrociti)negli uomini:da 5,4 a 6,0 milioni/mcl;nelle donne: da 4,8 a 5,4 milioni/mcl
–Emoglobina(Hb)negli uomini: da 13,5 a 17,5 g/dl;nelle donne:da 12,0 a 15,5 g/dl
–Ematocrito(Hct)negli uomini: da 38,8% a 50%;nelle donne: da 34,9% a 44,5%
–Piastrine: da 150.000 a 450.000 /mcl
È importante sottolineare che solo un medico può valutare se i valori dell’emocromo sono normali o no, in base ai segni/sintomi del paziente e ad altri esami di laboratorio.
Globuli rossi
I globuli rossi, o eritrociti, sono cellule a forma discoidale con un diametro di circa 7-8 micron e uno spessore di circa 2 micron. La membrana delle cellule è formata da un doppio strato di fosfolipidi e proteine, che contengono emoglobina, una proteina che lega e trasporta l’ossigeno dai polmoni ai tessuti del corpo. I globuli rossi sono prodotti dal midollo osseo ehanno una durata di circa 120 giorni. Il loro numero nel sangue è essenziale per il trasporto di ossigeno e la regolazione del pH del sangue. Inoltre, i globuli rossi sono in grado di deformarsi per passare attraverso i piccoli capillari e trasportare ossigeno anche ai tessuti più lontani. I globuli rossi o eritrociti hanno una struttura molto semplice e compatta, ideale per il trasporto dell’ossigeno attraverso il flusso sanguigno.I globuli rossi sono prodotti dal midollo osseoe hanno una vita media di circa 120 giorni, dopo di che vengonodistrutti nel fegato o nella milza. La loro produzione e distruzione è regolata da un ormone chiamato eritropoietina, prodotto soprattutto dal rene.
Globuli bianchi o leucociti
I globuli bianchi o leucocitisono lecellule del sanguedeputate alladifesa del nostro organismo.Sono prodotti dal midollo osseo e presenti anche nel sistema linfatico. I leucociti si distinguono in cinque categorie principali: ineutrofili, i linfociti, i monociti, gli eosinofili e i basofili.Ogni categoria ha una funzione specifica nel combattere i patogeni e altre sostanze dannose che entrano nel nostro organismo. I globuli bianchi, o leucociti, sono cellule del sistema immunitario responsabili della difesa dell’organismo da agenti patogeni come batteri, virus, funghi e cellule tumorali. Esistono diversi tipi di globuli bianchi, ognuno con una funzione specifica e una struttura diversa.
Tutti i globuli bianchi condividono la caratteristica di avere un nucleo, a differenza deiglobuli rossi che sono privi di nucleo.
Il nucleo dei globuli bianchi può avere una forma differente a seconda del tipo di cellula.
I globuli bianchi si dividono in due categorie:granulocitieagranulociti.
I granulociti sono così chiamati a causa della presenza di granuli nel loro citoplasma.
Esistono tre tipi di granulociti:
–Neutrofili: hanno il nucleo a forma di ferro di cavallo e granuli nella loro parte citoplasmatica. Sono responsabili della fagocitosi, ovvero dell’inglobamento e della distruzione di batteri e altri agenti patogeni.
–Eosinofili: hanno il nucleo a forma di ferro di cavallo e granuli grandi e arancioni nel loro citoplasma. Sono coinvolti nella risposta a parassiti e in alcune reazioni allergiche e infiammatorie.
–Basofili: hanno il nucleo a forma di ferro di cavallo e granuli di medie dimensioni bluastri nel loro citoplasma. Sono coinvolti nella risposta allergica e nella difesa contro i parassiti.
Gli agranulociti non hanno granuli nel loro citoplasma.
Si dividono in due categorie:
–Linfociti: hanno il nucleo rotondo o a forma di corno e sono responsabili della produzione di anticorpi e della risposta immunitaria a lungo termine.
–Monociti: hanno il nucleo a forma di fagiolo e sono responsabili della fagocitosi dei batteri e dell’eliminazione delle cellule morte e dei detriti cellulari.
Piastrine o trombociti
Le piastrine o trombociti, sono un tipo di cellule del sangue che svolgono un ruolo chiave nella coagulazione del sangue. Le piastrine vengono prodotte dal midollo osseo e si accumulano nella circolazione sanguigna per poi essere attivate quando si verifica una lesione al tessuto. In questo caso, le piastrine si uniscono tra di loro formando un tappo piastrinico che tampona la ferita e previene la fuoriuscita del sangue. Le piastrine sono anche importanti per la riparazione dei vasi sanguigni danneggiati e per la formazione di nuovi tessuti. Tuttavia, in alcune condizioni patologiche, come la trombocitopenia (riduzione del numero di piatrine nel sangue), la coagulazione può essere compromessa e potrebbe essere necessario un trattamento medico per prevenire o gestire il rischio di sanguinamento eccessivo.
Emocromo valori alti
Un emocromo con valori alti indica un aumento nel numero di alcuni componenti del sangue. Ad esempio,un alto numero di globuli bianchi può indicare un’infezione o una malattia infiammatoria, mentreun alto numero di globuli rossi può indicare una condizione di policitemia vera.. In ogni caso, è importante discutere i risultati dell’emocromo con il proprio medico per determinare la possibile causa e/o la necessità di ulteriori esami diagnostici.
Emocromo valori bassi
Se si riferisce al valore basso nell’emocromo di uno o più componenti del sangue, ci possono essere diverse cause, tra cui:
–Anemia: una riduzione dei livelli di globuli rossi nel sangue può portare a una diminuzione dell’emoglobina e del valore di ematocrito.
–Leucopenia: un basso numero di globuli bianchi può essere causato da problemi del sistema immunitario, infezioni, tumori, uso di farmaci impegnativi, ecc.
–Trombocitopenia: un basso numero di piastrine nel sangue può indurre un rischio maggiore di emorragia.
–Malattie del midollo osseo: alcune condizioni che riguardano il midollo osseo (come la mielofibrosi) possono portare a una carenza di una o più componenti del sangue.
–Carenze nutrizionali: una carenza di ferro, vitamine B12 o acido folico può causare anemia.
Per determinare la causa di un basso valore nell’emocromo è necessaria una valutazione più dettagliata, che può includere ulteriori esami ematici, esami del midollo osseo, ecografie, biopsie, ecc.
Come si esegue un emocromo?
Per eseguire un emocromo, un operatore sanitario prenderà un campione di sangue da una vena del braccio del paziente, utilizzando un ago sterile. Il campione di sangue verrà poi analizzato automaticamente mediante un analizzatore automatizzato o manualmente da un operatore sanitario, usando una microscopia.
L’emocromo è composto da varie parti, tra cui:
–La conta dei globuli rossi (eritrociti) e la misurazione dell’emoglobina e dei livelli di ematocrito
–La conta dei globuli bianchi(leucociti), compresi i neutrofili, i linfociti, i monociti, gli eosinofili e i basofili

I risultati dell’emocromo possono essere utili per diagnosticare e monitorare una vasta gamma di condizioni mediche, tra cui anemia, infezioni, malattie autoimmunitarie, cancro e problemi di coagulazione del sangue.
