Introduzione
Il virus dell’epatite A (HAV) è un virus a RNA lineare, non capsulato,della Famiglia Picornaviridae. Si tratta di un virus termostabile, resistente agli acidi e, soprattutto, all’attacco della bile, essendo privo di rivestimento lipidico, caratteristica questa che rende possibile la trasmissione fecale-orale. Viene inattivato dall’ebollizione per circa 1 min., dalla formaldeide, dal cloro e dall’esposizione ai raggi ultravioletti. In coltura cellulare non mostra effetto citopatico.
Il virus dell’epatite B (HBV) è un virus a DNA circolare, a doppia elica incompleta, appartenente alla famiglia Hepadnaviridae.
La particella virale completa è denominata particella di Dane e consiste di un involucro esterno (HB-sAg) e un nucleo centrale (core) che contiene il genoma virale, a sua volta costituito da 4 sequenze geniche, che codificano per la sintesi delle proteine virali. Sono state identificate numerose varianti di HBV; in base alle variazioni dell’antigene S (HBsAg) sono stati descritti 8 genotipi (indicati con le lettere dell’alfabeto dalla A alla H), con diversa distribuzione geografica. Importanti dal punto di vista clinico sono le varianti della regione pre-core, caratterizzate dalla mancata sintesi e secrezione di HBe-Ag, con conseguente attiva replicazione virale pur in presenza di anti-HBe nel siero.
Attualmente la maggior parte dei portatori cronici nel bacino del Mediterraneo (fino al 90% anche in Italia) e in Estremo Oriente (fino al 60 70%) ha selezionato varianti pre-core. Tali varianti determinano quadri infiammatori epatici più aggressivi, più frequenti forme fulminanti e maggiore resistenza alla terapia con interferoni. In corso di replicazione di HBV vengono immessi in circolo virioni completi (particelle di Dane) e un numero superiore di particelle sferiche o a bastoncino più piccole e costituite dalle sole proteine dell’involucro (HB-sAg), prive di capacità infettante, ma immunogeneticamente efficaci e in grado di legarsi agli anticorpi specifici e provocare patologie da complessi.
Dopo l’identificazione dei marcatori sierologici del virus HBV e le conseguenti misure di controllo di sangue ed emoderivati si rese evidente, dalla mancata scomparsa dei casi di epatite
post-trasfusionale, che doveva esistere un altro virus responsabile di tali infezioni epatiche. Il virus, denominato inizialmente non -A, non-B fu poi individuato e caratterizzato nel 1989 e denominato virus dell’epatite C (HCV). È un virus a RNA a singola elica, della famiglia Flaviviridae.
Epidemiologia
- Il virus dell’epatite A si trasmette quasi esclusivamente per via fecale-orale con l’ingestione di cibo o acqua contaminati da feci umane di soggetti con infezione acuta, in quanto l’eliminazione del virus con le feci è massima durante il periodo di incubazione fino alla comparsa dell’ ittero e non si stabilisce lo stato di portatore.
- Le vie di trasmissione del virus dell’epatite B sono la via parenterale classica (trasfusione di emoderivati, scambio di siringhe nei tossicodipendenti, punture accidentali negli operatori sanitari); la via parenterale inapparente (contaminazione di spazzolini da denti, rasoi, forbicine o di strumenti e oggetti utilizzati per tatuaggi e piercing; la trasmissione sessuale e la trasmissione verticale.
- La trasmissione del virus dell’epatite C avviene per via parenterale e parenterale inapparente, interessando soggetti delle categorie a rischio indicate per l’infezione da virus B. La trasmissione verticale non è frequente, essendo stimata intorno al 4-7% per le madri con viremia positiva, ma aumenta in caso di coinfezione con HIV fino al 15-20%. Anche il rischio di trasmissione sessuale non è ancora definitivamente chiarito: è stimato basso nelle coppie monogame, mentre non è trascurabile nei soggetti con elevata promiscuità sessuale, nei rapporti omosessuali e in presenza di coinfezione con HIV. Infine il rischio professionale (operatori sanitari e assimilabili) e la trasmissione in ambito sanitario sono bassi nei paesi a elevato sviluppo igienico-sanitario, ma salgono nei paesi in via di sviluppo, dove, per esempio, i donatori di sangue non vengono controllati per l’infezione da HCV e l’utilizzo di materiale monouso è ancora poco diffuso.
Quadri anatomopatologici
Le alterazioni istopatologiche caratteristiche dell’ epatite virale acuta consistono in aspetti degenerativi degli epatociti, in particolare la cosiddetta degenerazione balloniforme, con rigonfiamento cellulare e rarefazione del citoplasma, e la degenerazione eosinofila. A questo si associa una reazione infiammatoria con aumento del numero e delle dimensioni delle cellule di Kupffer, infiltrati linfomonocitari, localizzati negli spazi portali, ma, nelle forme a maggiore gravità, estesi anche nelle aree periportali. La necrosi epatocitaria è il reperto caratteristico dell’epatite acuta e presenta distribuzione focale, sebbene nelle forme ad andamento clinico più grave possa essere maggiormente estesa e confluente diffondendosi da uno spazio portale all’altro o
tra uno spazio portale e vena centrolobulare e creando ponti di necrosi, fino alla necrosi massiva con perdita della struttura lobulare epatica, sostituita da infiltrato infiammatorio e collasso dell’organo, come si osserva nei quadri di epatite fulminante.
Manifestazioni cliniche
Si riconoscono quattro fasi principali.
- Periodo di incubazione: è clinicamente silente e ha durata differente per i diversi virus epatici, variando da 15-50 giorni nell’ epatite A, a 30-180 giorni nell’epatite B e D, a 15 -80 giorni nella C e 15-60 giorni nell’epatite E.
- Periodo prodromico: precede l’ittero e dura da 3 a 7 giorni. È caratterizzato da una sintomatologia aspecifica: malessere, anoressia, nausea, vomito, astenia intensa, talvolta dolenzia all’ipocondrio destro. Alcuni sintomi in questa fase possono indirizzare il sospetto diagnostico verso l’uno o l’altro virus, essendo febbre, mialgie e brividi più frequentemente associati all’esordio dell’epatite da HAV, con un quadro simil-influenzale;in circa il 10% dei casi di epatite B sono presenti sintomi da “malattia da siero”, quali atrailgia, artrite, rash, febbre,poliarterite nodosa e, nei bambini, acrodermatite papulare o malattia di Gianotti Crosti, caratterizzata da eruzione eritematopapulare.
- Periodo itterico: dura in genere da 2 a 4 settimane ed è di intensità variabile, da assente (epatite anitterica) a minima (subittero) a intensa con valori elevati di bilirubina sierica cui si associano sintomi quali prurito, feci ipocoliche e urine ipercromiche.
- Periodo della convalescenza: è caratterizzato dalla progressiva scomparsa dell’ittero con risoluzione della sintomatologia e delle alterazioni biochimiche.
Oltre al quadro clinico classico esistono forme particolari che meritano una descrizione a parte.
- Epatite colestatica: in questo caso l’ittero è particolarmente pronunciato, è presente prurito e rialzo degli indici della stasi epatica e il decorso è solitamente protratto, sebbene benigno.
- Epatite fulminante: è la conseguenza drammatica e per lo più fatale della necrosi epatica massiva: si verifica in circa lo 0,1- 0,3 % dei casi di epatite A, nel l’1% dei casi di epatite B, eccezionalmente in corso di epatite C.
Terapia
La terapia dell’epatite acuta è perlopiù sintomatica; quale ne sia l’eziologia,si giova di un periodo di riposo assoluto e dieta leggera priva di grassi animali e basso contenuto proteico, con eventuale supporto di liquidi e glucosio per via endovenosa. Nei casi più gravi le misure dietetiche sono più restrittive, essendo consigliata una dieta ipoproteica al fine di ridurre il carico di azoto al fegato e, allo stesso fine, vengono utilizzati paramonicina, lattulosio ed enteroclismi, quali prevenzione delle encefalopatia porto-sistemica. L’unica terapia efficace della forma fulminante è il trapianto di fegato, per cui ogni paziente che presenta una simile evoluzione va segnalato a un Centro Trapianti. Non esistono terapie specifiche per l’epatite A. Gli studi condotti con lamivudina nelle forme gravi di epatite B hanno dato risultati incerti, sebbene l’ ottima tollerabilità ne suggerisca l’utilizzo.
Prevenzione
La prevenzione dell’infezione da HAV si basa sul miglioramento delle condizioni igienico-sanitarie nelle aree endemiche; sull’assunzione di alcune precauzioni alimentari per il viaggiatore in tali zone evitare (mitili, verdure crude, acqua non imbottigliata)
e soprattutto sull’ immunoprofilassi. Anche la profilassi dell’epatite B si avvale del miglioramento delle condizioni igienico-sanitarie e dell’educazione dei gruppi a rischio. Sono disponibili immunoglobuline per la profilassi passiva degli esposti a rischio (soprattutto neonati di madre portatrice o soggetti che hanno subito una puntura accidentale con materiale potenzialmente infetto), da somministrare sempre in associazione alla prima dose di vaccino. L’immunoprofilassi attiva nei confronti di HBV si ottiene mediante vaccino ricombinante, che contiene solo HBsAg e che induce la produzione di un titolo protettivo di anticorpi neutralizzanti. In Italia questa vaccinazione è divenuta obbligatoria per tutti i nuovi nati a partire dal 1991. Dal 2011 la vaccinazione nei confronti di HBV è stata introdotta nei programmi di immunizzazione dell’infanzia in 180 paesi.
