Il trattamento dipende dalle cause sottostanti e può includere la terapia farmacologica concorticosteroidioimmunosoppressorie laterapia plasmatica. In alcuni casi, è necessario anche il trapianto di midollo osseo.

Tutti questi fattori possono scatenare una risposta autoimmune del sistema immunitario del corpo e portare alla distruzione dei globuli rossi, causando anemia emolitica autoimmune.

–Affaticamento
–Debolezza
–Pallore
–Dispnea(difficoltà respiratoria)
–Tachicardia(aumento della frequenza cardiaca)
–Dolore addominale
–Gonfiore addominale
– Colesterolo elevato nelle urine (nel caso di anemia emolitica autoimmunitaria da abiotrofia a membrane)
–Sanguinamentodal naso o dalle gengive
–Febbre
–Mal di testa
–Aumento della sensibilitàalla temperatura fredda
–Ittero(ingiallimento della pelle e degli occhi)
–Osservazionedi schiuma nelle urine.
Diagnosi
1.Esame emocromocitometrico completo: può rivelare una riduzione dei globuli rossi (anemia), un aumento della bilirubina e delle reticolociti.
2.Test di Coombs diretto e indiretto: questi test rilevano la presenza di anticorpi contro i globuli rossi. Il test di Coombs diretto rileva gli anticorpi già legati ai globuli rossi, mentre il test di Coombs indiretto cerca gli anticorpi circolanti nel sangue.
3.Esame della funzionalità dei reni e del fegato: l’AIHA può causare elevati livelli di bilirubina, che possono danneggiare questi organi.
4.Biopsia del midollo osseo: può essere utile per determinare la causa dell’AIHA.
5.Studio immunologicoe diagnostico di altre patologie autoimmunitarie: l’AIHA è spesso associata ad altre malattie autoimmunitarie come il lupus eritematoso sistemico.
6.Esami per la ricerca di eventuali infezioni: alcune infezioni, come quelle da virus epatite, possono causare l’AIHA.
In base ai risultati di questi esami, il medico può formulare la diagnosi di AIHA e stabilire la migliore terapia.
Terapia per l’anemia emolitica autoimmune
La terapia perl’anemia emolitica autoimmune (AIHA) dipende dalla gravità della malattia. Se l’AIHA è moderata o lieve, il trattamento può consistere in semplici cure, come il riposo e la riduzione dell’attività fisica, la somministrazione di integratori di ferro o l’assunzione di steroidi come la prednisone.
Se l’AIHA è grave e non risponde alla terapia a base di steroidi, possono essere utilizzati altri farmaci immunosoppressori più potenti, come l’azatioprina o il rituximab. In alcuni casi, può essere necessaria la trasfusione di sangue o la rimozione del milza (splenectomia).
In generale, il trattamento dipende dal tipo e dalla gravità dell’anemia emolitica autoimmune e deve essere personalizzato da un medico specializzato in malattie del sangue (ematologo).
